Arrival
To ensure a full massage session and allow for a relaxed and unhurried experience, please arrive at least 10 minutes before your scheduled appointment. If late arrival is unavoidable, your appointment may be shortened by ending at the normal time out of fairness to following clients. The original treatment cost will be charged.
Cancellation/No Show
You may cancel your appointment without charge up to 24 hours prior to your appointment start time. Same day cancellations may be charged a fee of 50% of the full appointment price. If you do not call to cancel your appointment, you will be charged a No-show fee of 100% of the appointment price.
Informed Consent and Confidentiality
Prior to each massage session, the treatment plan will be discussed with you. At your first visit with us you will be asked to sign the Treatment Consent Form stating that you have read the information, understand and agree to comply with Myotherapy’s policies and procedures. Clients who have not visited the clinic for at least a year may again be asked to fill out this form to ensure we have your current medical status and information. We treat all client’s visits and medical/personal information as highly confidential. We will not discuss your session with anyone outside of our staff without your express permission.
Existing and New Medical Conditions
It is the responsibility of the client to keep the therapist informed of any medical treatment or prescriptions he/she is currently undergoing and to provide written permission from a physician, chiropractor, physical therapist, etc., that massage may be received.
Online Booking
In order to obtain your desired appointment, we recommend scheduling as far in advance as possible to ensure availability. Online booking is available for your convenience at www.myohealingmassage.com. To hold your reservation, a credit card is required at the time of booking. Payment will not be finalized until after your massage session. We require a credit card be on file as part of our cancellation policy. Please Note: Confirmation emails and/or texts are ALWAYS sent directly after appointments have been reserved. If you do not receive an email or text, please call (801) 484-7652 to confirm your appointment.
Scope of Practice
All professional massage therapists at the Myotherapy Healing Massage Clinic are licensed, insured, and adhere to the “Utah Massage Therapy Code of Ethics and Standards of Practice.”
Professional Boundaries
Sexual interaction or sexual discussion of any kind between the client and the massage therapist is NOT tolerated. For the protection of our therapists and clients, we reserve the right to end a massage session at any time and deny a client’s request to visit a specific therapist. In the event that a massage session is ended due to client inappropriate behavior, the client may be charged for the entire session.
Age Requirement
Any child under the age of 18 is required to have a parent or legal guardian sign a Minor Consent Form. If a child is under the age of 16, a parent or guardian is required to be present in the massage room or the waiting room for the duration of the session.
Insurance Billing
Our clinic does not offer direct billing to insurance companies. We can provide an itemized receipt for your session that you may submit to your insurance company for possible reimbursement. You must contact your health insurance provider to verify that massage therapy is a covered plan benefit and for instructions on how to submit your claim. You are responsible for full payment at time of service.
Membership Policy
Myotherapy Healing Massage Clinic reserves the right to update, change, or discontinue memberships at any time. Myotherapy Healing Massage Clinic reserves the right to terminate memberships at any time due to failed payments or inappropriate behavior by you or a secondary member. Excluding termination for inappropriate conduct, all unused massages will remain on your account for you to redeem or they may be converted into gift certificates of equal value. Benefits have no cash value and no refunds will be issued for unused benefits.
Gift Certificates
Myotherapy Healing Massage Clinic is not responsible for lost or stolen gift certificates. Gift certificates cannot be refunded or redeemed for cash. Gift certificates have no cash value. Gift certificates may not be used to purchase other gift certificates. Myotherapy reserves the right to refuse service or reverse any gift certificate purchase for any reason and return the funds to the purchaser at the original purchase price. If the service price exceeds the amount of the gift certificate, the redeemer is responsible for the remaining balance. Gift certificates cannot be combined with ANY additional discounts, including: discounted gift certificates, coupons, membership discounts or special offers.
